Bicycling is often recommended for people with knee problems. Use
lower gears (real bike) or low resistance (stationary bike). A
recumbent bike is recommended for people with back problems, although
an upright bike with straight handlebars might work, too. In any case,
be sure that the bike parameters are fitted properly and that she
avoids side-to-side movement as she pedals. That is, use good form.
Depending on what her back problems are, she might be able to mitigate
or avoid them by doing exercises that strengthen the core, especially
side and back muscles. That would enable her to do other exercises.
First, she should probably see a doctor to be sure that the back
problems are not serious to begin with. Second, she should concentrate
on form first, not results -- as should everyone, but especially
someone who starts with problems.

That makes it really difficult to lose body fat. Losing fat is all
about "calorie deficit": burning more calories than consumed. But she
already consumes a low amount. Unless she has a huge amount of free
time, it is not likely that she can burn 500-600 calories more per day
through exercise, which is what it would take for her to lose a pound
of fat per week. That's okay: it just means it will take longer than
she might expect. I am guessing that she wants to lose 10-15 lb of
fat.
Unless she gets her body fat very very low, there is no guarantee that
she will lose the fat around her middle.
