I have both. The elbow pain is in both arms and the distal biceps tendon
pain is the left arm only.

I think a balance is probably good yes? Because I do no forearm exercises, I
try to grip everything without aids. My grip has improved over the past 12
months. I used to struggle with 40kg in each hand and now I can handle
(just) doing shrugs with 70kg in each hand.

I don't squat. I have a 'growth' in my left lower back which the GP called a
fat deposit. As soon as tried squatting, I had pain. So I didn't do them. I
do leg press instead which doesn't seem to cause any lower back issues. I
don't use the 45 at my gym as it has a very odd setup. The foot plate pivots
and the mounting point is very high.When you get the weight down low, it
seems to get heavier than the normal 45 which doesn't pivot. I was at my
bro's gym last Sunday and used their 45. It felt a lot better than the one
at my gym. I managed 150kgx12, 200kgx10 250kgx8 300kgx4(with a spot).

I realise that just about all of those machines are not 1:1 because the
weight stack is not directly connected to the grip point. I would have
thought it was about what you say. Have you actually taken scales into the
gym to obtain the ratio of various machines? It would be interesting.

No, i use elementary school mathematics.

Measure the distance of the handles from beginning to end position.

Measure the distance of the point where the weights are stacked.

Divide distance weight/distance handles.

If thats .5 then you moved .5 of the weight you assumed you were moving.

Very easy.
