Really depends on how you state your goal. My goal is to exercise for
35-45 min at high intensity (75% MHR and higher) [1]. So I would not
include the warm-up and cool-down in that time. Likewise, I do not
include the 1 min or so that it takes to move from one aerobic machine
to another. But if my goal were to spend 40 min "exercising", that
might include all time spent, including warm-up, cool-down and time
between machines.

[1] I do not believe there is any single standard relating terms like
low, moderate and high intensity to %MHR. I just learned that the ACSM
considers "moderate" to be 55-69%, "hard" is 70-89%, and "very hard" is
80% and above. I do not agree with that breakdown. But who am I to
question the ACSM?
